Requirements:
- Birth Certificate
- Marriage Certificate (If married)
- 1×1 picture (5pcs)
- Passport size picture (4pcs)
- Transcript of Records (College graduate/level)
- Form 137 (Highschool graduate)
- Medical Certificate
